gMS® Financial Assistance Plan
For uninsured patients, we have a Financial Assistance Plan to assist you with the cost of gMS® tests. Call us at (800) 643-4906 for more information.
The billing process: As the sole provider of gMS® tests, we will submit a claim to your insurance. Your insurance company may send the payment and/or the Explanation of Benefits (EOB) for this service directly to you.
If you receive a payment from your insurance provider, please endorse the check on the back and indicate, “Pay to the Order of Glycominds”. Please mail the check and a copy of the EOB or correspondence that accompanied the payment in the envelope provided.
On occasion, some insurance companies have been known to send a bill to the patient for these tests. If you receive a bill from your insurance provider, call us at (800) 643-4906 to ensure that the payment for your tests is processed by Glycominds directly.
*gMS® Co-pay Advantage Plan™ is open to both new and existing patients who are residents of the US or Puerto Rico and who have private insurance coverage. The offer is not valid for patients covered in whole or in part by Medicaid, Medicare, TRICARE, or any other federal or state government medical assistance plan or program (regardless of whether a specific test is covered), or by private health benefit programs that reimburse for the entire cost of laboratory tests. Use of this offer must be consistent with the terms of any benefit provided by health insurer, health plan, or private third-party payor. This offer is void in Massachusetts or where otherwise prohibited by law, taxed, or restricted. No additional purchase is required. This offer is valid only through participating patient service centers and may be changed or discontinued at any time without notice. This program is not health insurance. |
